You have to expect harsh replies in a satisfaction survey. Take a more personal approach to get a more honest, first hand, answer. "How could we have done better, for you?"

I think the first thing to recognise is that you risk insulting your customer by asking them to put time into a survey which is for YOUR benefit. You should never do this. Cabe is right - talk to your customer and in such a way that there is benefit for the customer. If you can't be bothered to talk to a customer then don't waste his time - by sending out a survey that takes you far less time per customer than you are expecting them to spend you are in effect syaing that your time is more valuable.
I had an examnple the other day, the HAAS rep dropped in to give me a calendar for 2012, and have a chat. He found out what he needed to know and I got a bit of market intellignece about local prospects for me. Mutually beneficial, much more honest and open (in the sense of 'open' question) and neither of us ever said anything so crass as "would you buy from us again".
I get bombarded with customer surveys, most I ignore and those I don't I answer somewhere between honestly and totally tongue in cheek according to my mood.
